ABSTRACT

Introdução: Zumbido é um sintoma crescente na população pediátrica e sua multifatoriedade etiológica demanda amplas investigações para utilizar-se adequadas intervenções. Objetivos: relatar os efeitos de uma abordagem não medicamentosa em um caso de remissão total do zumbido pediátrico oriundo da disfunção tubária. Método: Este artigo apresenta um indivíduo com sete anos, do sexo feminino e com queixa de zumbido crônico associado a disfunção tubária, considerado escasso na literatura. Além disso, este estudo descreve as avaliações médicas (neurológica e otorrinolaringológica), a avaliação audiológica e psicoacústica do zumbido, o processo diagnóstico e a intervenção fonoaudiológica realizada. Utilizou-se uma abordagem não medicamentosa que contemplou exercícios miofuncionais orofaciais, Manobra de Valsalva e limpeza nasal. Resultados: Após um mês de intervenção, com a prática diária dos exercícios, o indivíduo estudado referiu ausência da percepção do sintoma e das demais queixas auditivas. Este resultado também foi constatado nas avaliações audiológicas. Conclusão: A abordagem não medicamentosa da disfunção tubária demonstrou bons resultados frente ao manejo do zumbido crônico, para o presente caso. (AU)


Introduction: Tinnitus is a growing symptom in the pediatric population and its etiological multifactorial demands extensive investigations to use appropriate interventions. Objective: report the effects of a non-medicated approach of a case of total remission of pediatric tinnitus from tube dysfunction. Methods: This article presents a seven-year-old female patient with a complaint of chronic tinnitus associated with tubal dysfunction, considered rare in the literature. Besides that, this study describes the medical evaluations (neurological and otorhinolaryngological), the audiological and psychoacoustic evaluations of the tinnitus, the diagnostic process and the speech-language intervention performed. It was used a non-medication approach that included orofacial myofunctional exercises, Valsalva maneuver and nasal cleaning. Results: After a month of intervention, with the daily practice of exercises, the studied subject reported the absence of perception of the symptom and other auditory complaints. This result was also verified in the audiological evaluations. Conclusion: The non-medication approach to tubal dysfunction has shown good results in relation to the management of chronic tinnitus for the present case. (AU)


Introducción: Acufeno es un síntoma creciente en la población pediátrica y su multifatoriedad etiológica demanda amplias investigaciones para utilizar adecuadas intervenciones. Objetivos: Informar los efectos de un enfoque no farmacológico de un caso de remisión total del acufeno pediátrico oriundo de la disfunción tubárica. Metodos: Este artículo presenta un sujeto con siete años, del sexo femenino y con queja de acufeno crónico asociado a disfunción tubárica, considerado escaso en la literatura. Además, este estudio describe las evaluaciones médicas (neurológica y otorrinolaringológica), la evaluación audiológica y psicoacústica del zumbido, el proceso diagnóstico y la intervención fonoaudiológica realizada. Se utilizó un abordaje no medicamentoso que contempló ejercicios miofuncionales orofaciales, Maniobra de Valsalva y limpieza nasal. Resultados: Después de un mes de intervención, con la práctica diaria de los ejercicios, el sujeto estudiado refirió ausencia de la percepción del síntoma y de las demás quejas auditivas. Este resultado también se constató en las evaluaciones audiológicas. Conclusión: El abordaje no medicamentoso de la disfunción tubárica demostró buenos resultados frente al manejo del acufeno crónico, para el presente caso. (AU)

Introdução: O zumbido é uma ilusão auditiva consciente, uma sensação sonora não relacionada com uma fonte externa de estimulação. Objetivos: Caracterizar a Acufenometria, Limiar Diferencial de Mascaramento, o questionário de qualidade de vida Inventário de Desvantagem do Zumbido e Potencial Auditivo de Tronco Encefálico em adultos normo-ouvintes com zumbido, com a finalidade de comparar seus achados. Método: Vinte indivíduos do sexo feminino e masculino, entre 20 e 60 anos de idade, normo-ouvintes com queixa de zumbido, foram submetidos ao Acufenometria, Limiar Diferencial de Mascaramento, Inventário de Desvantagem do Zumbido e Potencial Evocado Auditivo de Tronco Encefálico. Resultados: AAcufenometriarevelou que o pitch médio foi de 4,3 KHz à orelha direita e 4,6 KHz à orelha esquerda. O loudness médio foi de 21,7 dBNS à orelha direita e 23,5 dBNS à orelha esquerda. O Limiar Diferencial de Mascaramento médio mostrou-se alterado. O Inventário de Desvantagem do Zumbido médio correspondeu à classificação de grau leve. O Potencial Evocado Auditivo de Tronco Encefálico apresentou parâmetros dentro da normalidade bilateralmente. Conclusão: Constatou-se que adultos normo-ouvintes com queixa de zumbido apresentam zumbido de pitch agudo bilateral com discreto impacto na qualidade de vida, condução adequada das vias auditivas até o tronco encefálico e comprometimento na identificação de sons na presença de ruído, demonstrando que o zumbido pode ter repercussões nas habilidades auditivas centrais. Introduction: Tinnitus is a conscious auditory illusion, a sound perception unrelated to any external stimulus source. Objectives: To characterize the Acuphenometry, Masking Level Difference, the quality of life questionnaire Tinnitus Handicap Inventory and Auditory Brainstem Response in normal hearing adults with tinnitus, with the purpose of comparing the findings. Method: Twenty female and male individuals, between 20 and 60 years of age, normal hearing with complaints of tinnitus, underwent Acuphenometry, Masking Level Difference, Tinnitus Handicap Inventory and Auditory Brainstem Response. Results: The Acuphenometry showed the average pitch was 4.3 KHz to the right ear and 4.6 KHz to the left ear. The average loudness was 21.7 dBSL to the right ear and 23.5 dBs to the left ear. The average Masking Level Difference was altered. The average Tinnitus Handicap Inventory corresponded to the classification of mild grade. Auditory Brainstem Response showed parameters within normal range bilaterally. Conclusion: It was found that normal hearing adults with tinnitus complaints have bilateral acute pitch tinnitus with a slight impact on quality of life, appropriate conduction of auditory pathways to the brainstem and impaired identification of sounds in the presence of noise, demonstrating that tinnitus can have repercussions on central auditory skills. Introducción: El tinnitus es una ilusión auditiva consciente, una sensación de sonido no relacionada con una fuente externa de estimulación. Objetivos: Caracterizar la coincidencia de tono y volumen, el umbral de enmascaramiento diferencial, el inventario de minusvalía para acúfenos y el potencial auditivo del tronco encefálico en adultos normoyentes con acúfenos, con el fin de comparar sus hallazgos. Método:Veinte sujetos masculinos y femeninos, con edades entre 20 y 60 años, audición normal con tinnitus, fueron sometidos a acúfenos, Umbral de Enmascaramiento Diferencial, Inventario de Desventajas de Tinnitus y Potenciales Evocados Auditivos del Tronco Encefalico. Resultados: La combinación de tono y volumen reveló que el tono promedio era de 4,3 KHz en el oído derecho y de 4,6 KHz en el oído izquierdo. Mientras que el volumen medio fue de 21,7 dBNS para el oído derecho y de 23,5 dBNS para el oído izquierdo. Se modificó el umbral diferencial de enmascaramiento promedio. El Inventario de Desventajas de Tinnitus promedio correspondió a la clasificación de grado leve. El Potenciales Evocados Auditivos del Tronco Encefalico presentó parámetros dentro del rango normal bilateralmente. Conclusión:Se encontró que los adultos normooyentes con quejas de tinnitus presentan tinnitus de tono alto bilateral con leve impacto en la calidad de vida, conducción adecuada de las vías auditivas al tronco encefálico y deterioro en la identificación de sonidos en presencia de ruido, demostrando que Tinnitus puede tener repercusiones en las habilidades auditivas centrales. ABSTRACT

El tinnitus o acúfeno es la percepción de sonido en ausencia de un estímulo acústico externo; es uno de los motivos de consulta más comunes en el área de otorrinolaringolo­gía. Se ha asociado a diversas etiologías tales como ototoxicidad, tumores, traumatismo craneoencefálico y trastornos psiquiátricos, siendo también las causas de carácter idio­pático ampliamente conocidas. Las características clínicas de los tinnitus son variables de paciente a paciente, por lo que su diagnóstico, tratamiento y seguimiento son un desafío clínico Si bien recientes estudios han propuesto distintos manejos clínicos y terapéuticos, estos continúan hoy en día en estudio. Esta revisión narrativa pretende realizar una actualización sobre el abordaje diagnóstico, clínico y terapéutico de los tinnitus en la última década.


Tinnitus, the perception of sound in the absence of an external acoustic stimulus, is one of the most common reasons for consultation in the area of otorhinolaryngology. It has been associated with various etiologies such as ototoxicity, tumors, head trauma, and psychiatric disorders; idiopathic causes are also widely known. The clinical characteristics of tinnitus are variable from patient to patient; therefore, its diagnosis, treatment, and follow-up are a clinical challenge. Although recent studies have proposed different clinical and therapeutic approaches, these are still under analysis today. This narrative review aims to update the diagnostic, clinical and therapeutic approach to tinnitus in the last decade.


Tinnitus ou zumbido é a percepção do som na ausência de um estímulo acústico externo; é um dos motivos mais comuns de consulta na área de otorrinolaringologia. Tem sido associada a várias etiologias, como ototoxicidade, tumores, traumatismo craniano e distúrbios psiquiátricos, sendo também amplamente conhecidas causas idiopáticas. As características clínicas do zumbido são variáveis de paciente para paciente, por isso seu diagnóstico, tratamento e acompanhamento são um desafio clínico, embora estudos recentes tenham proposto diferentes abordagens clínicas e terapêuticas, ainda hoje estão em estudo. Esta revisão narrativa visa atualizar a abordagem diagnóstica, clínica e terapêutica do zumbido na última década.

ABSTRACT

RESUMO Objetivo Verificar a relação entre zumbido e aptidão cardiorrespiratória em pessoas após COVID-19. Métodos Estudo transversal com amostra de pessoas pós-COVID -19 que responderam à Escala Visual Analógica para zumbido e questionário padronizado contendo dados sobre internação e zumbido. Para avaliar a aptidão cardiorrespiratória, utilizou-se a avaliação clínica e o Teste de Bruce para mensurar o consumo de oxigênio diretamente (via analisador de gases, com utilização do consumo pico de oxigênio - VO2pico). Resultados Participaram 192 pessoas, com média de idade de 47,8 ± 12,6 anos. A prevalência de zumbido autorreferido foi de 27,1% (n = 52). Dos 52 pacientes com zumbido, 27 iniciaram com o sintoma durante ou após o diagnóstico de COVID-19. Houve diferença significativa para o VO2pico absoluto entre os grupos com e sem zumbido (p = 0,035), sendo que o grupo com zumbido apresentou os menores valores; o tamanho do efeito foi pequeno. Não houve correlação entre os escores da Escala Visual Analógica para o zumbido e os valores de VO2pico absoluto e relativo (p > 0,05). Conclusão Houve diferença estatisticamente significativa entre as queixas de zumbido e o VO2pico nas pessoas após a COVID -19, sendo que o grupo com zumbido apresentou o VO2pico absoluto menor do que o grupo sem zumbido. Nos pacientes com zumbido, também foi encontrado VO2pico absoluto e relativo menor para as mulheres, além do VO2pico relativo menor para os hipertensos e obesos.


ABSTRACT Purpose To verify the relation between tinnitus and cardiorespiratory fitness among people after COVID-19. Methods Cross-sectional study with a sample of people post Covid-19 who responded to the Visual-Analog Scale for tinnitus and standardized questionnaire containing data on hospitalization and tinnitus. To evaluate cardiorespiratory fitness, it used the clinical assessment and Bruce test to measure oxygen consumption directly (via gas analyzer, using peak oxygen consumption - VO2peak). Results In total, 192 participants were assessed, with a mean age of 47.8 ± 12.6 years . The prevalence of self-reported tinnitus was 27.1% (n = 52). Of these 52 patients with tinnitus, 27 people started with the symptom during or after the diagnosis of COVID-19. There was a significant difference for the absolute VO2peak and the groups with and without tinnitus (p = 0.035): the tinnitus group showed the lowest values, the effect size was small. There was no correlation between the scores of the Visual-Analog Scale for tinnitus and the absolute and relative VO2peak (p > 0.05). Conclusion There was a statistically significant difference between tinnitus complaints and the VO2peak among people post COVID-19, with the tinnitus group having a lower absolute VO2peak than the non-tinnitus group. In tinnitus patients, we also found lower absolute and relative VO2peak for women, in addition to lower relative VO2peak for hypertensive and obese patients.

RESUMO Objetivo identificar os fatores associados a zumbido, percepção auditiva e risco de distúrbio vocal entre jogadores de jogos digitais. Métodos estudo transversal realizado com jogadores de jogos digitais por meio de questionários eletrônicos para coletar dados relacionados à saúde auditiva e vocal. Foram analisados três desfechos principais: zumbido, percepção auditiva (sensação da diminuição da audição) e risco de distúrbio de voz, mensurado pela Escala de Sintomas Vocais. As variáveis independentes referiram-se às características sociodemográficas, perfil dos jogadores e sinais e sintomas. Aplicaram-se os testes Qui-quadrado de Pearson e Exato de Fisher. Resultados a amostra foi composta por 59 jogadores, sendo a maioria do gênero masculino (79,7%), com idade entre 18 e 24 anos (69,5%). Com relação aos desfechos, 27,6% dos participantes apresentaram zumbido, 57,6%, risco de distúrbio vocal e 13,6% referiram sensação de diminuição da audição. Observou-se associação entre zumbido e cefaleia (p=0,024), zumbido e uso de smartphone (0,012), sensação de diminuição da audição e presença de ansiedade (p=0,049). O risco de distúrbio de voz associou-se ao maior tempo de horas jogadas por semana (p=0,020). Conclusão a autopercepção de zumbido esteve associada à cefaleia e ao uso de smartphone. Houve, ainda, associação entre ansiedade e sensação de diminuição da audição, além de risco de distúrbio de voz e maior tempo de horas jogadas por semana.


ABSTRACT Purpose to identify factors associated with tinnitus, auditory perception and risk of voice disorders among digital game players. Methods cross-sectional study carried out with digital game players through electronic procedures to collect data related to hearing and vocal health. Three main outcomes were analyzed: tinnitus; auditory perception (sensation of diminished hearing) and risk of voice disorders, measured by the Vocal Symptoms Scale (VSS). The independent variables were related to sociodemographic characteristics, player profile and signs and symptoms. Pearson's chi-square test and Fisher's exact test were applied. Results the sample consisted of 59 players. Most were male (79.7%) and ages between 18 and 24 years (69.5%). Regarding the terms, 27.6% had tinnitus, 57.6% were at risk of vocal disorder and 13.6% reported hearing loss. There was an association between tinnitus and headache (p=0.024), tinnitus and smartphone use (0.012), hearing loss and anxiety (p= 0.049). A risk of voice disorder(s ) was associated with more hours played per week (p=0.020). Conclusion self-perceived tinnitus was associated with headache and smartphone use; there was also an association between anxiety and the feeling of hearing impairment, in addition to the risk of voice disorders and more hours played per week.

RESUMO Objetivo caracterizar o grau de intensidade e desconforto do zumbido de indivíduos, pós-COVID-19. Métodos pesquisa observacional, exploratória e de corte transversal com 242 participantes, divididos em dois grupos iguais, pareados de acordo com gênero e faixa etária: grupo-controle (GC), composto por indivíduos com zumbido em período pré-pandêmico, e grupo de estudo (GE), composto por participantes que referiram zumbido pós-COVID-19, de diferentes regiões do país. Foram comparados os escores gerais e de domínios específicos do Tinnitus Handicap Inventory (THI) e da Escala Visual Analógica (EVA), por meio de análise estatística descritiva, testes de normalidade, comparação de grupos e de correlação entre as variáveis. Resultados em cada grupo, 93 participantes eram do gênero feminino e 28, do masculino, sendo a média das idades de 35 anos. Os graus desprezível e leve do THI e leve e moderado da EVA foram maiores para o GE, enquanto o domínio catastrófico do THI e a EVA apontaram piores escores para o GC. Houve diferença significativa entre todos os domínios das escalas inter e intragrupos. Conclusão manifestações mais brandas de incômodo, intensidade e desconforto do zumbido pós-COVID-19 foram encontradas, em comparação ao sintoma decorrente de outras causas. Assim, o impacto do zumbido para a população estudada foi menos autorreferido, apontando para melhores possibilidades terapêuticas e prognósticas.


ABSTRACT Purpose to characterize the degree of intensity and discomfort of tinnitus in post-COVID-19 individuals. Methods observational, exploratory and cross-sectional research with 242 participants, divided into two equal groups matched according to sex and age group. The control group, composed of individuals with tinnitus in the pre-pandemic period, and the study group, composed of participants who reported post-COVID-19 tinnitus in different regions of the country. General and domain-specific scores from the Tinnitus Handicap Inventory (THI) and Visual Analogue Scale (VAS) were compared through descriptive statistical analysis, normality tests, comparison of groups and correlation between variables. Results in each group, 93 participants were female and 28 male, with a mean age of 35 years. The "negligible" and "mild" degrees of the THI and the "mild" and "moderate" of the VAS were higher for the SG, while the catastrophic domain of the THI and the VAS indicate worse scores for the CG. There was a significant difference between all domains of the inter- and intra-group scales. Conclusion milder manifestations of post-COVID-19 tinnitus disturbance, intensity and discomfort were found, compared to symptoms arising from other causes. Thus, the self-reported impact of the studied population was lower, pointing to better therapeutic and prognostic possibilities.

RESUMO Objetivo analisar o impacto do distanciamento social devido à pandemia de COVID-19, sob a percepção e o incômodo com o zumbido crônico e a relação dos achados com a ansiedade e a depressão. Métodos a amostra foi composta por conveniência, incluindo sujeitos com idades de 21 a 78 anos. A avaliação foi realizada em dois períodos: pré-distanciamento (final de 2019) e durante o distanciamento (2020). A segunda avaliação foi realizada de forma online, via Google Forms, consistindo nos seguintes procedimentos: questionário semiestruturado, Tinnitus Handicap Inventory (THI), Inventário de Ansiedade de Beck (BAI) e Inventário de Depressão de Beck (BDI). Resultados nas comparações dos procedimentos entre os períodos pré e pós-distanciamento social, foi evidenciada diferença estatisticamente significativa para o THI e o BAI, mas não para o BDI. Na correlação da percepção do zumbido crônico com a ansiedade, foi observada diferença estatisticamente significativa, positiva e forte, mas não significativa com a depressão. Conclusão o distanciamento social devido à pandemia de COVID-19 piorou a percepção e o incômodo com o zumbido crônico, estando correlacionado com aumento nos sintomas de ansiedade.


ABSTRACT Purpose To analyze the impact of social distancing, due to the COVID-19 pandemic, on the perception and nuisance with chronic tinnitus and the relationship of the findings with anxiety and depression. Methods The sample was composed by convenience, including subjects aged 21 to 78 years. The evaluation was conducted in two periods: pre-distancing (late 2019) and during distancing (2020). The second evaluation was conducted online via Google Forms, consisting of the following procedures: Semistructured questionnaire, Tinnitus Handicap Inventory (THI), Beck Anxiety Inventory (BAI), and the Beck Depression Inventory (BDI). Results In the comparison of the results of the procedures performed pre-social distancing and during social-distancing, a statistically significant difference was evidenced for the THI and the BAI, but not for the BDI. In the correlation of the perception of chronic tinnitus with anxiety, a statistically significant difference was observed, positive and strong, but not significant with depression. Conclusion Social distancing, due to the COVID-19 pandemic, worsened the perception of and annoyance with chronic tinnitus and was correlated with an increase in anxiety symptoms.

ABSTRACT

Objective: To clarify the long-term characteristics of tinnitus following treatment of sudden deafness and its long-term physical and mental effects on patients. Methods: A retrospective analysis was performed on 88 patients (46 males and 42 females; Age from 11 to 89 years) with sudden deafness treated in Department of Otoscope Surgery of Peoples's Libration Army General Hospital in Beijing from April 2020 to January 2021, and the occurrence of tinnitus and treatment effect of all patients were analyzed. Follow-up was conducted for patients with residual tinnitus after treatment for more than 1 year by the investigation and filling in the survey information collection form, Tinnitus Evaluation Questionnaire (TEQ) and Tinnitus Handicap Inventory (THI). Descriptive statistics and SPSS 22.0 software were used for statistical data analysis. Results: In this study, 93.2% (82/88) of patients with sudden deafness were accompanied by tinnitus at the onset, and the proportion of long-term tinnitus after treatment was 90.2% (74/82). After 1 year of treatment for sudden deafness, the improvement of tinnitus was significant in low-frequency sudden deafness compared with those of high-frequency, flat and total deafness sudden deafness (χ2 value was 6.801, 4.568 and 4.038, all P<0.05). In patients with residual tinnitus, 9 (12.2%) patients felt minimal loudness or even no loudness, 34 (46.0%) patients felt slight loudness, 28 (37.8%) patients felt tinnitus was relatively loud, and 3 (4.1%) patients felt tinnitus was loud or noisy. Nine (12.2%) patients's sleep was often affected, 41 (55.4%) patients's sleep was sometimes affected, 9 (12.2%) patients's sleep was rarely affected, 15 (20.3%) patients's sleep was almost not affected. Twenty-eight (37.8%) patients basically completely adapted to tinnitus and 46 (62.2%) patients did not completely adapted to residual tinnitus. Eight (10.8%) patients had no impact on life, 39 (52.7%) patients had slight impact, 22 (29.7%) patients had moderate impact, and the other 5 (6.8%) patients had greater impact. According to tinnitus evaluation questionnaire(TEQ), there were 12 cases (16.2%) of grade Ⅰ, 26 cases (35.1%) of grade Ⅱ, 28 cases (37.8%) of grade Ⅲ, 7 cases (9.5%) of grade Ⅳ and 1 case (1.4%) of grade Ⅴ. According to tinnitus handicap inventory(THI), tinnitus disability was classified into grade Ⅰ, 22 cases (29.7%), grade Ⅱ, 14 cases (18.9%), Grade Ⅲ, 27 cases (36.5%) and grade Ⅳ, 11 cases (14.9%). Conclusion: The rate of residual tinnitus following treatment of sudden deafness is high. Some of the patients can completely adapt residual tinnitus after one year, but some of them will be affected when sleep, work and study. Residual tinnitus can lead to tinnitus disability in different degrees.

ABSTRACT

Objective: To analyze the clinical characteristics and treatment of middle ear myoclonus. Methods: Fifty-six cases of middle ear myoclonus were enrolled in Shandong Provincial ENT Hospital, Shandong University from September 2019 to August 2021, including 23 males and 33 females. The age ranged from 6 to 75 years, with a median age of 35 years; Forty-seven cases were unilateral tinnitus, nine cases were bilateral tinnitus. The time of tinnitus ranged from 20 days to 8 years. The voice characteristics, inducing factors, nature (frequency) of tinnitus, tympanic membrane conditions during tinnitus, audiological related tests, including long-term acoustic tympanogram, stapedius acoustic reflex, pure tone auditory threshold, short increment sensitivity test, alternate binaural loudness balance test, loudness discomfort threshold, vestibular function examination, facial electromyography, and imaging examination were recorded. Oral carbamazepine and/or surgical treatment were used. The patients were followed up for 6-24 months and the tinnitus changes were observed. Results: Tinnitus was diverse, including stepping on snow liking sound, rhythmic drumming, white noise, and so on. The inducing factors included external sound, body position change, touching the skin around the face and ears, speaking, chewing and blinking, etc. Forty-four cases were induced by single factor and 9 cases were induced by two or more factors. There was no definite inducing factor in 1 case. One patient had tinnitus with epilepsy. One case of traumatic facial paralysis after facial nerve decompression could induce tinnitus on the affected side when the auricle moved. Tympanic membrane flutter with the same frequency as tinnitus was found in 12 cases by otoscopy, and the waveform with the same frequency as tinnitus was found by long-term tympanogram examination. There were 7 patients with no tympanic membrane activity by otoscopy, the 7 cases also with the same frequency of tinnitus by long-term tympanogram examination, but the change rate of the waveform was faster than that of the patients with tympanic membrane flutter. All patients with tinnitus had no change in hearing. One case of tinnitus complicated with epilepsy (a 6-year-old child) was treated with antiepileptic drug (topiramate) and tinnitus subsided. One case suffered from tinnitus after facial nerve decompression for traumatic facial paralysis was not given special treatment. Fifty-four cases were treated with oral drug (carbamazepine), of which 10 cases were completely controlled and 23 cases were relieved; 21 cases were invalid. Among the 21 patients with no effect of carbamazepine treatment, 8 patients were treated by surgery, 7 patients had no tinnitus after surgery, 1 patient received three times of operation, and the third operation was followed up for 6 months, no tinnitus occurred again. The other 13 cases refused the surgical treatment due to personal reasons. Conclusions: Middle ear myoclonus tinnitus and the inducing factors manifestate diversity. Oral carbamazepine and other sedative drugs are effective for some patients, and surgical treatment is feasible for those who are ineffective for medication.

RESUMO Objetivo analisar as respostas do processamento das informações auditivas em sujeitos adultos jovens com transtorno do zumbido. Métodos estudo de caráter descritivo e quantitativo. Participaram do estudo 36 sujeitos, com idades entre 19 e 35 anos, divididos em dois grupos: Grupo 1 (20 sujeitos com zumbido crônico) e Grupo 2 (16 sujeitos sem zumbido). Todos os indivíduos foram submetidos à anamnese, inspeção visual do meato acústico externo, audiometria tonal liminar, logoaudiometria, medidas de imitância acústica e, como instrumentos de avaliação, os testes comportamentais do processamento auditivo central - Teste Dicótico de Dígitos, Teste Padrão de Frequência, Teste de Fala no Ruído, Masking Level Difference e Gap in Noise. Resultados existiram diferenças estatisticamente significativas entre os grupos para as respostas do Teste Dicótico de Dígitos na orelha esquerda e para o Gap in Noise em ambas as orelhas. Conclusão adultos jovens com zumbido crônico apresentam prejuízo nas habilidades de integração binaural e resolução temporal.


ABSTRACT Purpose Objective: To analyze auditory information processing responses in young adult subjects with tinnitus disorder. Methods A descriptive and quantitative study was conducted, involving 36 subjects aged between 19 and 35 years. The subjects were divided into two groups: Group 1 (20 subjects with chronic tinnitus) and Group 2 (16 subjects without tinnitus). All participants underwent medical history assessment, Visual Inspection of the External Auditory Canal, Pure-Tone Audiometry, Speech Audiometry, Acoustic Immittance Measures, and used Behavioral Central Auditory Processing Tests as an evaluation tool - Dichotic Digits Test (TDD), Frequency Pattern Test, Speech in Noise, Masking Level Difference, and Gap in Noise (GIN). Results Statistically significant differences were found between the groups for TDD responses in the left ear and for GIN in both ears. Conclusion Young adults with chronic tinnitus exhibit impairments in binaural integration and temporal resolution skills.

ABSTRACT

Tinnitus refers to the perception of abnormal sound in the absence of external sound stimulation. It can have an impact on a person's mood, memory, attention, and mental state, although the mechanism of tinnitus is still unclear. In recent years, the research on the central neural mechanism of tinnitus has attracted the attention of scholars.Functional magnetic resonance imaging (fMRI),as an effective imaging technology, has been actively employed in this field. This paper provides a systematic summary of studies on the central neural mechanism of tinnitus by fMRI in recent years,revealed the changes of functional connections among tinnitus-related neural networks,such as auditory network,limbic system,default mode network and salience network. The central neural mechanism of tinnitus involves multiple networks that interact with each other. By understanding this mechanism, we hope to develop more targeted prevention and treatment strategies to help patients alleviate long-term tinnitus.

ABSTRACT

Patients with patulous Eustachian tubes(PET) usually suffer from annoying symptoms, such as tinnitus, autophony and aural fullness, due to the excessive opening of the Eustachian tube. There is no uniform standard of treatment, and conservative therapy combined with"Stepup"surgical intervention strategy is the main treatment. In this article, we reviewed various surgical treatments of patulous Eustachian tube in recent years, including key points of surgical operation, effectiveness, safety and complications. Full communication and evaluation are needed to establish appropriate patients' expectations preoperatively. A "Stepup" treatment strategy will be carried out, including conservative treatment, tympanic membrane surgery, Eustachian tube pharyngeal orifice constriction surgery, Eustachian tube tympanic orifice plug surgery and Eustachian tube muscle surgery, which aims to maintain normal Eustachian tube function and good middle ear ventilation.

ABSTRACT

A 27-year-old female patient suffering endolymphatic sac tumor with intralabyrinthine hemorrhage was reported. The patient had hearing loss in the left ear with continuous tinnitus, and MRI showed the soft tissue shadow of endolymphatic sac. Considering that the tumor involved semicircular canal and vestibule,endolymphatic cyst tumor resection was performed by labyrinth route. After surgery, there was no cerebrospinal fluid leakage and facial nerve function was normal. More importantly, enhanced MRI of temporal bone showed no tumor recurrence 1 year after surgery.

ABSTRACT

Contexto: A Terapia Notch surgiu como estratégia promissora no controle do zumbido tonal. Trata-se de estratégia que utiliza a plasticidade neural para inibir as frequências laterais àquela do zumbido, equilibrando sinapses excitatórias e inibitórias e, por conseguinte, reduzindo a percepção e incômodo do zumbido. Objetivo: Avaliar a efetividade da Terapia Notch no controle do zumbido tonal. Material e métodos: Trata-se de sinopse baseada em evidências. Procedeu-se à busca por estudos que associavam zumbido e Terapia Notch em quatro bases eletrônicas de dados: Cochrane - Central de Registros de Ensaios Clínicos - CENTRAL (2022), PubMed (1966-2022), Portal BVS (1982-2022) e Embase (1974- 2022). Dois pesquisadores independentemente extraíram os dados e avaliaram a qualidade dos estudos para a síntese. O desfecho primário de análise envolveu a melhora clínica do zumbido. Resultados: Foram encontradas 538 referências. Seis estudos foram incluídos (n = 233 participantes). Discussão: A literatura apresenta poucos estudos clínicos de avaliação da terapia Notch para zumbido. Trata-se de ensaios clínicos com amostragem reduzida e heterogeneidade alta. Embora o nível de evidência seja ainda muito limitado, todos esses estudos concluem que a terapia Notch pode ser efetiva no controle do zumbido tonal. Conclusões: A terapia Notch pode trazer benefícios no controle do zumbido tonal, mas novos ensaios clínicos de qualidade e bom delineamento são necessários para maior robustez da evidência.

ABSTRACT

Resumen La fisiopatología del tinnitus crónico no pulsátil es poco clara, pero se reconoce un componente psicológico relevante, por tanto, existen distintas aproximaciones psicoterapéuticas que han sido estudiadas. La terapia cognitivo conductual es la que cuenta con mayor evidencia. Ella contempla la reestructuración de las cogniciones disfuncionales que favorecen la presencia del tinnitus. Se ha verificado su eficacia en los niveles de distrés, calidad de vida, severidad, sintomatología depresiva e insomnio asociados. La desensibilización y reprocesamiento por movimientos oculares recoge algunos presupuestos de la terapia cognitivo conductual, pero considera técnicas como la estimulación bilateral. Sus resultados en patologías crónicas somáticas han sugerido la aplicación en tinnitus, corroborando su eficacia en calidad de vida, distrés y sintomatología depresiva. Dos psicoterapias basadas en mindfulness aplicadas en tinnitus son la reducción del estrés basado en el mindfulness y la terapia cognitiva basada en mindfulness. La primera ha demostrado eficacia en la calidad de vida y en la disminución y el refuerzo de cogniciones negativas y positivas, respectivamente. La segunda, es eficaz en la severidad, intensidad, distrés, ansiedad, depresión y discapacidad asociadas a tinnitus. Finalmente, la terapia de aceptación y compromiso promueve la aceptación como componente central del tratamiento de condiciones crónicas, considerando el control que ejerce el contexto sobre la vivencia de estas condiciones y el compromiso terapéutico. Esta terapia es eficaz en calidad de vida. Se promueve la investigación en aproximaciones psicoterapéuticas para el tinnitus, lo que posibilitará la aplicación de terapias más específicas y la clarificación de su fisiopatología.


Abstract The pathophysiology of non-pulsatile chronic tinnitus is unclear, but it is recognized a relevant psychological component. In this sense, different psychotherapeutic approaches have been studied. Cognitive behavioral therapy is the psychotherapy with the most evidence. It considers the restructuring of dysfunctional cognitions that favor the presence of tinnitus. Its efficacy has been verified on distress, quality of life, severity, associated depressive symptoms and insomnia. Eye movement desensitization and reprocessing includes some conceptions of cognitive behavioral therapy and considers techniques such as bilateral stimulation. Its results in chronic somatic pathologies have suggested its application in tinnitus, corroborating its efficacy in quality of life, distress, and depressive symptoms. Two mindfulness-based psychotherapies applied in tinnitus are mindfulness-based stress reduction and mindfulness-based cognitive therapy. The first has shown efficacy on quality of life and reduction and reinforcement of negative and positive cognitions, respectively. The second is effective on severity, loudness, distress, anxiety, depression and disability associated with tinnitus. Finally, acceptance and commitment therapy promote acceptance as a central component of the treatment of chronic conditions, considering the control exerted by the context over the experience of these conditions and the therapeutic commitment. This therapy is effective on quality of life. We foster the research on psychotherapeutic approaches to tinnitus, which will make it possible the application of more specific interventions and, at the same time, elucidate its pathophysiology.

Introducción: El tinnitus es una condición de alta prevalencia que puede afectar la calidad de vida. Objetivos: Evaluar la calidad de vida y sus factores asociados en pacientes adultos con tinnitus. Métodos: Estudio analítico transversal. La población incluida fueron pacientes adultos con síntoma de tinnitus, atendidos en el servicio de otorrinolaringología en un hospital de referencia en Lima, Perú. El efecto del tinnitus sobre la calidad de vida fue medida mediante el cuestionario Tinnitus Handicap Inventory. Resultados: Se incluyeron 151 pacientes con tinnitus; 31,2 por ciento fueron varones y la media de edad fue 66 años, 17,2 por ciento y 23,1 por ciento de pacientes con tinnitus tuvieron antecedentes de trastornos depresivos y desórdenes de ansiedad, respectivamente; 85,4 por ciento y 62,2 por ciento de pacientes con tinnitus presentaron hipoacusia y cefalea, respectivamente. El Tinnitus Handicap Inventory tuvo una mediana de 26 (rango intercuartil: 8-50), un valor mínimo de 0 y un valor máximo de 100. De los pacientes, 101 (66,8 por ciento) tuvieron una discapacidad leve a moderada debido al tinnitus y 50 (33 por ciento) discapacidad moderada a severa. En el análisis multivariado se encontró que la ansiedad (OR = 2,59; IC 95 por ciento 1,14-5,9; p = 0,02), hipoacusia (OR = 12,9; IC 95 por ciento 1,65-102,1; p= 0,01) y cefalea (OR = 2,61; IC 95 por ciento 1,18-5,8; p = 0,01) se asociaron con discapacidad moderada a severa debido al tinnitus. Conclusiones: Los pacientes adultos con tinnitus frecuentemente tuvieron una afectación moderada a severa de su calidad de vida. Los factores asociados a discapacidad moderada a severa debido a tinnitus fueron la ansiedad, cefalea e hipoacusia(AU)


Introduction: Tinnitus is a highly prevalent condition that can affect the quality of life of patients. Objective: To assess the quality of life and its associated factors in adult patients with tinnitus. Methods: Cross-sectional and analytical study carried out with a population that included adult patients with tinnitus symptoms attended in the otorhinolaryngology service of a reference hospital in Lima, Peru. The effect of tinnitus on quality of life was measured using the Tinnitus Handicap Inventory. Results: The study included 151 patients with tinnitus: 31.2 percent were male and the mean age was 66 years, while 17.2 percent and 23.1 percent of patients with tinnitus had a history of depressive disorders and anxiety disorders, respectively. Of tinnitus patients, 85.4 percent and 62.2 percent had hearing loss and headache, respectively. The Tinnitus Handicap Inventory showed a median value of 26 (interquartile: 8-50), a minimum value of 0, and a maximum value of 100. Of the patients, 101 (66.8 percent) had mild to moderate disability due to tinnitus, while 50 (33 percent) had moderate to severe disability. Contexto: O zinco é um elemento-traço que age no sistema auditivo, atuando em sinapses auditivas e na cóclea, sobretudo junto à enzima superóxido dismutase. Objetivo: Avaliar a efetividade da suplementação de zinco para o tratamento de cocleovestibulopatias. Material e Métodos: Trata-se de sinopse baseada em evidências. Procedeu-se à busca por estudos que associavam zinco à perda auditiva, tontura e zumbido em três bases eletrônicas de dados: Cochrane - Central de Registros de Ensaios Clínicos - CENTRAL (2022), PubMed (1966- 2022) e Portal BVS (1982-2022). Dois pesquisadores independentemente extraíram os dados e avaliaram a qualidade dos estudos para a síntese. Os desfechos de análise envolveram melhora de perda auditiva, tontura e zumbido. Resultados: Foram encontrados 231 estudos. Cinco estudos (quatro ensaios clínicos randomizados e uma revisão sistemática) envolvendo um total de 249 pacientes. Discussão: A literatura mundial apresenta poucos estudos relacionando zinco e cocleovestibulopatias. A maioria trata-se de estudos in vitro ou de experimentação animal. Os estudos em humanos são ensaios clínicos de baixa amostragem e elevada heterogeneidade, que avaliaram melhora de perda auditiva e melhora de zumbido. Nenhum estudo avaliou melhora da tontura. O nível de evidência é muito baixo e não permite, nesse momento, aferir a efetividade do zinco para tratamento de cocleovestibulopatias em humanos, sendo sua utilização clínica baseada na experiência de cada profissional. Conclusões: Não há evidência de efetividade da suplementação de zinco no tratamento de cocleovestibulopatias, sendo recomendada a realização de novos estudos de boa qualidade metodológica. Resumen La patología del sistema nervioso central, habitualmente, no provoca síntomas auditivos unilaterales, ya que la vía auditiva central está formada por una red de conexiones cruzadas entre los diferentes núcleos que la forman. Además, hay que considerar que una lesión pequeña puede extenderse a más de una estructura provocando varios déficits neurológicos debido a la proximidad de los tractos y núcleos nerviosos. Las lesiones unilaterales circunscritas en el colículo inferior son infrecuentes. No obstante, se han descrito casos en los que lesiones unilaterales de diversas etiologías en esta localización causaban síntomas auditivos. Ya que la vía auditiva central es cruzada, síntomas auditivos detectados con más frecuencia afectaban concretamente a la capacidad de localización del sonido o la comprensión verbal. Presentamos el caso de un hombre de 44 años con acúfeno unilateral derecho de larga evolución, sin otra clínica asociada quien fue diagnosticado de un tumor en el colículo inferior derecho mediante resonancia magnética cerebral. Se exponen los hallazgos clínicos y radiológicos del caso.


Abstract Central nervous system diseases usually do not cause auditory symptoms because the central auditory pathway consists on a network of crossed connections between the different nuclei that form it. In addition, we must consider that a small lesion might extend to more than one structure producing many neurologic symptoms due to the proximity of tracts and nuclei in the midbrain. Unilateral circumscribed lesions at inferior colliculus are rare. Nevertheless, lesions at this location causing auditory symptoms have been described. Because of the crossed central auditory pathway, the most commonly detected auditory symptoms specifically affected the ability to locate sound or verbal comprehension. We present the case of a 44-year-old man with a long-term monoaural right-sided tinnitus without other complaints who was diagnosed of a tumour at right inferior colliculus by neuroimaging. Clinical and radiological findings of this case are discussed.
